Synopsis:
In this webinar, Sarah Cole will be sharing her insight and experiences of how 4PCN in South Gloucestershire

1. Has created its care coordination service with a focus on anticipatory care and patients with learning disabilities and severe mental illness.
2. Has included people with lived experience into their workforce and the training they have provided.
3. Works in collaboration with other parts of the system.

We will also discuss:
1. How the PCN and practice dynamic has changed over the years.
2. The challenges evidencing impact, outcome and value for money.
3. How they plan to aid staff progression in their care coordination team.

Speakers:
Tara Humphrey, MBA
Tara Humphrey holds an MBA in Healthcare Leadership and Management and is the Founder and CEO of THC Primary care. Tara is also a leading healthcare communicator who has written over 200 blogs on leadership and management and primary care networks, she is published in the London Journal of Primary Care, and hosts the Business of Healthcare Podcast, where she has released over 240 episodes.

Sarah Cole, PCN Manager
Sarah is a full-time mum. In her spare time, she is a PCN Manager in South Gloucestershire, and the National PCN Representative for the Institute of General Practice Management. She has 28 years’ experience working in health care management, 21 of those being in primary care. She is also the Non-Clinical Lead for Community Mental Health in South Gloucestershire.
